CASID from CARD/PMCT

With an option in the ToH-Config menu you can control, if the CASID is
taken from card as normaly or if it should be taken from the PMCT.
You can do the following selections with the "CAS Preselect":

CARD|CAS1|CAS2|CAS3|CAS4|CAS5|CAS6

There are the following new entries in the PMCT:

CAS1 0000 1800 1722 0500 0100 0B00 4A60 => For Emulatorek/Wallbanger auto switch (force 1722)
CAS2 0000 1800 1762 0500 0100 0B00 4A60 => For Emulatorek/Wallbanger auto switch (force 1762)
CAS3 0000 1800 1762 0500 0100 0000 0000 => For people with SkyCrypt and Conax supported CAM's
CAS4 0000 0500 1762 0100 0604 0606 0608 => For people with Nagra supported CAM's or Irdeto 2 test
CAS5 0000 0D00 0D01 0D02 0D03 0D04 0D05 => For Cryptoworks test
CAS6 0000 0918 0960 0961 09A4 09D0 0E00 => For NDS Videoguard test

This entries have to be set in the CSV file:
